About JourneyCare

The largest nonprofit provider of palliative, supportive and end-of-life care in Illinois, JourneyCare serves nearly 3,000 adult and pediatric patients and their families each day with an unmatched depth and breadth of expertise, experience and services, providing exceptional care for each person, and support for those who love them, at every stage of the journey.
JourneyCare was formed in 2015 as a result of the merger of three highly respected nonprofit community-based healthcare providers, Horizon Hospice & Palliative Care, JourneyCare, and Midwest Palliative & Hospice CareCenter, each with more than 30 years of experience. The merger expanded access to a wide range of services and care to even more patients and families across 10 counties in the Chicago area and northern Illinois.
JourneyCare helps people facing serious illness live life to the fullest, with dignity and without pain, on their own terms. We care for the whole person—body, mind and spirit—not just the disease, and work together with the patient, family and other care partners to create a customized care plan that supports and respects their goals, preferences and choices.

Current Opportunities at JourneyCare
CNA Assist Volunteers
CNA Assist Volunteers are paired with a Certified Nursing Assistant and help with a patient’s “activities of daily living,” such as feeding, bathing, changing linens, etc. Specialized training is provided to help prepare volunteers to provide this tender care to our patients.

Hospitality Volunteers
Hospitality Volunteers assist JourneyCare staff with clerical tasks such as answering phones and call lights, faxing important documents, providing tours of the hospice unit, and so on.

Care Companions
Care Companions visit with patients and families and provide an extra layer of support through simple conversation, playing a game, watching television, etc. They volunteer at the Care Centers or in the home.
